What You’ll Do:
- Provide compassionate hospice care in patients’ homes
- Act as Case Manager – coordinate with patients, families & care team
- Educate and support patients & families on comfort care
- Work independently with the support of a dedicated team
What We’re Looking For:
- Active RN License (CA)
- 2+ years of nursing experience as an RN (hospice/home health/skilled nursing preferred)
- Compassion, communication & care-first mindset
- Valid driver’s license/reliable transportation.
